I have read and understand the application guidelines and confirm that the information provided by me is true and correct. If successful for a Māori Trades Training Scholarship, I acknowledge and agree to the following conditions:

1. Actively attend and engage in my programme of study
2. My progress will be monitored by WelTec staff and if any issues arise, they will be discussed with me
3. On completion of my programme of study with WelTec, I intend to enter into employment and will be committed to ongoing training via a New Zealand apprenticeship or cadetship

The personal information provided in my application is collected by and will be held by Whitireia (Whitireia Community Polytechnic Ltd.), DX Box: SX33459, 3 Wi Neera Drive, Porirua 5022 and/or WelTec (Wellington Institute of Technology Ltd.) whose address is Private Bag 39803, Lower Hutt 5045. The information provided in my application will be used for the purpose of administering and assessing my application. Failure to provide all the information requested may mean the selection panel is unable to fully consider my application.

My name may appear in reports and publicity material published by WelTec and/or Whitireia or government agencies if I am successful in receiving a scholarship. Whitireia and WelTec have agreements with Ngāti Toa Rangatira, Te Āti Awa and Mafutaga Faifeau Samoa to provide work mentoring. Contact information may be shared with these organisations to enable support to be provided. I have a right under the Privacy Act 2020 to obtain access to and request correction of any personal information held by WelTec and/or Whitireia regarding myself.
